An initial investigation report has been released about the Air India plane crash that took place in Ahmedabad, India, on June 12. The report has raised serious questions about what caused the accident, calling one key action during the flight “mysterious.”

According to the report, both of the aircraft's engine fuel control switches were found to be in the "cut-off" position instead of "run." Normally, pilots switch to "cut-off" only after a plane has safely landed. But in this case, there was no sign of any emergency, technical failure, or pilot communication that would explain why this was done mid-air.

The report confirmed that the aircraft had no mechanical or software problems. Fuel tests were also done, and the fuel quality was found to be completely normal. This makes the situation even more puzzling, as nothing so far explains why the engines were shut down while the plane was still in the air.

The crash involved an Air India Boeing 787-8 Dreamliner and caused major destruction. Of the 241 people on board, 260 were reported dead — which may be a reporting error. In addition, five buildings on the ground were damaged.

A full investigation is still ongoing.
